Overview Responsible to the Oncology Service Line Director. Works with adult oncology patients with a site-specific diagnosis (i.e. breast cancer, prostate cancer, etc. ) while also willing to help any patient presenting/referred with a need. Collaborates with the interdisciplinary care team to guide and assist the patients/families through the cancer care continuum. Responsible for complex cases, patient/family education, supportive care triage/resource referral, support services referrals, and family support. Works collaboratively with physicians, clinicians and the leadership team to ensure that patients receive timely, appropriate and high quality care. Responsible for on-going development of the site-specific program and functions as an active part of the Oncology team for marketing and planning, performance improvement, care path development, and physician relations. What you will do Identifies and tracks quality indicators specific to the Navigation program. Ensures standardized quality indicators are tracked and analyzed across the navigation program. Demonstrates strong communication skills and ability to work with diverse populations of patients and caregivers. Develops and maintains relationships with key referring and practicing physicians for the identified population and continually works with physicians for care coordination. Works effectively and harmoniously with co workers and other members of the care team. Guides and assists adult oncology patients and their caregivers through the entire disease process, providing education, support, referrals, and care coordination as needed. Works closely with Cancer Services and the Cancer Registry Staff to identify and track indicators of the entire cancer program, such as the NQF. Works to continually develop and improve the Navigation program. Works with Ad Hoc Committee, physician and community leaders to assess resource and program needs. Identifies areas for improvement within the department and cancer program at large. Works towards creative, effective and efficient solutions. Involved with, and active in, relevant community groups and organizations, such as the American Cancer Society, etc. Develops and implements a site-specific patient navigation program tailored to the needs of the assigned patient population. Actively works with Cancer Services team to identify, plan, promote, and implement appropriate community education and awareness. Promotes Riverside Cancer Care Services through physician relations. Facilitates medical staff participation in quality management and program leadership. Attends Oncology Committee, Cancer Conferences, Chart Rounds and other committees as appropriate. Demonstrates knowledge of, and appropriately refers patients to, community resources for support and assistance.
