***Sign on Bonus of $600 (Half paid out at 3 months and other half at 12 months by meeting the hour requirement)
Job Title: Case Manager
Reports To: Program Manager
FLSA Status: Exempt
Summary: Maintains intensive case management and supervision of youth referred to the shelter for youth by performing the following duties.
Duties and Responsibilities: include the following. Other duties may be assigned.
Ensures clients are compliant with court orders and remain law abiding.
Meets with clients and families frequently once placed in Brittany’s Place and monitors client progress as shelter resident, at school, and in the community.
Identifies client and family needs and makes appropriate referrals to other county or community agencies.
Creates comprehensive case plans and transition plans that reflect the needs of clients and families.
Documents client contact and service coordination in an accurate and timely manner.
Works effectively and collaboratively with other professionals in the field.
Transports or arranges transportation for clients to medical, court, or other appointments.
Maintains knowledge of culturally specific services available in the community.
Maintains knowledge of the juvenile justice system, relevant laws, regulations and policies regarding social services provisions, reporting requirements, data practices, and client rights.
Maintains flexibility to work evenings, holidays, and weekends as needed to meet the needs of clients and families.
Demonstrates effective teamwork and collaboration with management and co-workers.

Education/Experience:
Bachelor's degree (B.A.) from a four-year college or university; one to two years related experience and/or training; or equivalent combination of education and experience.
