Scheduling needs:
- Shifts available Saturday and Sunday 9am-5pm
- Additional shifts may be available in another nearby home
Responsibilities:
- Provide nursing care to patients in their homes.
- Conduct thorough assessments of patients to identify and address their medical, emotional, and social needs.
- Administer medications and treatments as prescribed by physicians, and monitor patient response.
- Educate patients and their families on medical conditions, treatments, and self-care techniques.
- Document patient care in accordance with agency policies and standards.
- Provide toddler care, including administering medications, feeding, and bathing, as needed.
- Maintain accurate and up-to-date medical records, including medical documentation and reporting of patient outcomes.
- Monitor patient progress and adjust care plans as needed to ensure optimal health outcomes.
- Maintain a safe and healthy environment, adhering to infection control practices and promoting patient safety.

Qualifications:
- Current LPN license in the state of New Jersey.
- Minimum of 1 year of experience as an LPN, preferably in home health or a related field.
- Excellent communication and interpersonal skills.
- Experience with Trach, Vent, G-Tube
