Responsibilities:
- Provide direct support and care to individuals with developmental disabilities, dementia, and other complex needs in various settings, including nursing homes, home care, and long-term care facilities.
- Develop and implement individualized care plans that meet the unique needs and preferences of each client.
- Assist with daily living activities, such as bathing, dressing, grooming, and personal care.
- Provide memory care support, including managing medications and monitoring cognitive and emotional changes.
- Promote independence, self-advocacy, and community involvement by encouraging clients to engage in meaningful activities and pursue their interests.
- Collaborate with multidisciplinary teams to ensure comprehensive care and support.
- Maintain accurate records of client progress, including documenting care plans, medications, and any notable changes or concerns.
- Provide emotional support and companionship to clients and their families.
Experience:
- Previous experience working with individuals with developmental disabilities, dementia, or other complex needs is essential.
- Experience in caregiving, home care, or long-term care settings is highly valued.
- Knowledge of care plans and the ability to develop and implement personalized care plans is a significant asset.
- A strong understanding of dementia care principles and memory care techniques is required.
- Ability to work in a fast-paced environment and adapt to changing situations is necessary.
- High school diploma or equivalent is preferred.
