Job Summary
The Behavioral Health Care Coordinator plays a vital role in supporting patients with mental health needs by facilitating access to appropriate resources and services. This position involves collaboration with healthcare providers, DFPS, and their families to ensure comprehensive care and support. The ideal candidate will possess strong organizational skills and a solid understanding of medical terminology, particularly within the behavioral health field.
Duties
- Coordinate patient care by scheduling appointments, managing follow-ups, and ensuring timely communication between patients and healthcare providers.
- Maintain accurate patient records and documentation in compliance with minimum standards.
- Assist patients in navigating the healthcare system, including referrals to specialists and community resources.
- Write progress notes, incident reports & other documentation as required. Abide by safety and education plans, managing correspondence, and handling inquiries.
- Collaborate & communicate with treatment team & co-workers to develop individualized care plans that address child's behavioral health needs.
- Educate patients and families about treatment options, available resources, and self-care strategies.
- Monitor patient progress and report any concerns to the Supervisors, Administrator, & healthcare professionals as required according to policy and minimum standards.
Skills
- Proficient in supervising children diagnosed with DSM 5 Disorders and other behavioral challenges.
- Strong knowledge of how to de-escalate aggressive behavior without corporal punishment or power struggle methods.
- Excellent communication skills, both verbal and written, for effective interaction with children and treatment teams.
- Ability to manage multiple tasks efficiently while maintaining attention to detail and provide line of sight supervision
- Experience working in a GRO or RTC with children
- Compassionate approach to child care with a focus on empathy and understanding of mental health challenges.
Job Types: Full-time, Part-time, PRN
Pay: $14.00 - $18.00 per hour
Expected hours: 40 per week
