RESPONSIBILITIES
The responsibilities of the RN field nurse in our programs include but not limited to:
- Completes initial homehealth assessment of patient/family to determine home care needs, completes physical assessment of current and previous illnesses.
- Completes nursing assessments such as neuro, skin, vitals, and overall health assessment, reviews all potential participants in the Group Adult Foster Care Program.
- Conducts home visits as assigned, assessing the patient’s condition and progress toward goals.
- Coordinates the implementation of physician’s orders with client and Program Personnel.
- Conducts Oasis assessments
- Coordinates all contacts with physicians and certified home health agencies and PCA agencies as needed.
- Completes all necessary nursing documentation, including OASIS/MDS/CDS, nursing progress notes after patient assessment.
- Participates with in the planning, implementation and evaluation of participants, service plans, and other program components.
- Report changes in the member’s condition to the member’s physician. Acts as a liaison with area hospitals and different community agencies as needed.
- Provide patient education regarding hygiene, safety, and health concerns.
- Supervises the LPNs/CNA/HHA and or PCAs in the Program.
Salary: Negotiable, to be discussed at the interview.

Pay: $80,000.00 - $90,000.00 per year
