Position: Home Health Intake Coordinator (In-Person)

Key Responsibilities

• Manage the intake process for new home health patients, ensuring all necessary documentation is completed accurately and timely.
• Coordinate with healthcare providers, patients, and families to facilitate smooth transitions into home health care.
• Utilize home health or hospice software for patient management and documentation.
• Schedule physical therapy (PT), occupational therapy (OT), and skilled nursing services as needed.
• Maintain detailed and organized patient records.
• Communicate effectively with team members and external partners to ensure optimal patient care.
• Provide exceptional customer service and support to patients and their families.
  
  

Qualifications

• Previous experience in a home health intake or similar role.
• Proficiency in Kinnser software is highly desirable.
• Strong organizational and multitasking skills.
• Excellent communication and interpersonal skills.
• Being bilingual is a plus.
• Self-starter with a commitment to helping others.
• Dependable and dedicated to providing high-quality care.
• Bright personality with the ability to build personal relationships with the medical community.
• Ability to coordinate services to benefit the families and patients we serve.
• Willingness to dig in and make a conscious decision to stay and grow through the challenges and triumphs of working with a small but competitive company.
• Ability to multitask and work in a fast-paced environment without feeling defeated or taking stages of the grieving process personally.
• Experience in Hospice coordination a strong plus.
