The Home Care Nurse / Director of Nursing is responsible for the clinical aspects of the Health Care Service Firm
Responsibilities include :
- Perform clinical assessments of new and current clients in order to develop thorough and accurate care plans in accordance with state regulations and company policies. Revise care plans as necessary.
- Manage, prioritize and assign case load with Assessment nurses
- Ensure care plans and caregiver medical forms are accurately maintained in CRM system and client files.
- Assign tasks and track adherence in CRM system so caregivers can report daily activity.
- Consults with client, family and Caregivers to provide a safe and efficient patient care and review progress.
- Board of Nursing standards for CHHAs. Assess all newly hired CHHAs level of clinical competency.
- Supervise and evaluate performance of CHHAs based on care provided to client and family.
- Responsible for annual competency and performance evaluations of Home Health Aides to enhance the quality of care.
- Reflect the core values of A Place At Home at all times.
- Maintain a valid NJ RN license.
- Participate in staff meetings, track assessments and report on all pertinent issues in a timely manner.
