AbilityPath is hiring a Part Time and Full Time Independent Living Skills Case Manager to join the team! Positions available in Palo Alto and Santa Cruz!
The Independent Living Skills Case Manager/Instructor promotes empowerment and independent living for individuals who may have physical and/or intellectual disabilities through home and community based training and supports. Promote each individual’s overall goal to achieve complete independence

ABOUT THE ROLE
- Provide case management for participants on caseload. Assist participants with managing their finances, housing, grocery shopping, and scheduling medical appointments.
- Provide direct support to participants in learning their goals to either further or maintain their independence.
- Develop and Implement participant’s annual Individual Service Plans (ISP) with independent living goals.
- Develop participant’s quarterly progress report, and submit to state Regional Centers.
- Schedule meetings and report progress quarterly to the state Regional Centers.
- Provide daily documentation of participant sessions, mileage reimbursements, etc
- Must have reliable be able to travel to various sites
- Analyze situations or tasks and break down into teachable components according to individual training needs
- Maintain monthly communication with individuals concerned with participant including family, service coordinator and others
- Identify opportunities for increased independence and proactively address potential barriers to independence
- Be prepared to employ First Aid and/or CPR in emergency situations as needed
- Comply with participant’s rights, AbilityPath and program policies, CARF standards, NPS compliance, CCL regulations, CAL-OSHA, quality assurance standards, etc.
- Must act as a Mandated Reporter, and comply with applicable federal, state, and local regulations and laws
- Ability to provide engaging instruction to participants with various developmental levels, skill sets, and behaviors
- Ability to respond to challenging behaviors and support behavioral plans to increase engagement
- Awareness of, sensitivity to, and competence in supporting people with developmental disabilities
- Other duties as assigned
EDUCATION AND WORK EXPERIENCE:
Any combination of education and experience that would likely provide the required knowledge, skills and ability is qualifying, unless otherwise specified.
- Associate’s or Bachelor’s Degree in relevant field
- Experience working with adults with developmental disabilities
Hiring Requirements:
- Valid California Driver’s License/ID
- Clean Driving Record (if applicable)
- Proof of negative TB test results
- Verification of physical clearance
- Passing a criminal fingerprint clearance
- 18 years of age or job permit clearance
- Proof of Covid-19 Vaccination
PHYSICAL ACTIVITY:
- Ability to physically respond to adults with developmental disabilities
- Ability to lift/support at least 40 lbs as needed
- Ability to sit, stand, stoop, and bend frequently
- Ability to walk for extended periods of time and support adults with developmental disabilities on community outings
- Ability to drive company vehicles to transport participants
- Ability to sit and keyboard as needed
