What are the responsibilities and job description for the Case Manager position at Abundant Faith Christian Center?
Summary: The Case Manager provides supportive services to individuals and families. They are responsible for mentoring and guiding clients and linking them to community resources that will assist them in achieving individual and family goals and move them toward self-sufficiency.
Essential Functions:
- Make referrals and provide advocacy based on needs assessed through individual assessments
- Provide crisis intervention
- Assist individuals and families (residential and nonresidential) with food, rent and/or utility assistance
- Meet with clients to complete and follow-up on individual service plans
- Provide emergency or necessary transportation
- Provide clients with referrals, advocacy, and follow-up services needed to achieve client goals
- Maintain client notes, contact forms and referral logs
- Provide on-call availability to clients for emergencies
- Provide basic office duties, support and phone coverage
- Perform other duties as assigned with consideration given to priority of clients
- Assist with the development of client training and life skills workshops
- Monitor and assure compliance of supportive housing programs
- Screen applicants to ensure qualification of housing program
- Attain/copy proof of qualifying homeless/move-in documentation
- Monitor and assure compliance of the AFSS Residential Program Manual
- Update HUD Service Point database system on a daily basis
- Enter/Exit client information in system
- Assign client to appropriate unit
- Update client services and case notes in system
- Provide weekly update reports to director regarding clients verbal and/or written
Qualifications:
- Knowledge, skills and mental development equivalent to the completion of two years of college and/or experience in social services or related field
- Ability to communicate and work well with parents, children, staff and community agencies
- At least 21 years of age
- Ability to lift at least 40 pounds
- Must have current and valid Driver's License, reliable transportation, proof of car insurance
- Willingness to learn and uphold Abundant Faith Ministry's Policies and Procedures
- Ability to adequately complete programs records and reports, organize time and materials effectively
- Understanding and experience with Microsoft office computer programs
