Patient Services (Health Benefits)

The Patient Services role is essential to provide Access Community Health Centers (Access) patients with comprehensive clinical care and ensure better health outcomes. This position collaborates with other clinical staff to assess patient needs and coordinate services. The main function of the Patient Services role is to provide case management to at-risk patients and/or prenatal patients and coordinate necessary follow-up care. This position is also responsible for tracking and reporting activities related to case management and referrals.

  • Health Benefits: Helps patients with applications for Medical Assistance and the Affordable Care Act and administers Access's sliding discount program


Qualifications: Desired candidate may meet a combination of the following qualifications

  • A minimum of a high school diploma or equivalent and health care experience and/or social services experience or associates degree in health care, social services, or related field
  • Basic computer skills to be able to send and receive email and use programs such as Microsoft Excel, Word, and Access
  • Experience in case management
  • Experience in health benefits counseling
  • Experience in women’s health issues and/or working with a prenatal population
  • Experience with diverse cultures
  • Previous experience using Epic medical software
  • Fluent in Spanish and English

Compensation and Benefits:
Our total compensation package includes competitive wages, plus a full range of attractive benefits for regular full and part time staff working a minimum of 20 hours per week and includes:

1.0 FTE (40 hours weekly)
Hours: Monday-Friday, 9:00a-5:00p

Pay range begins at $23.75 hourly and is adjusted according to experience
