What are the responsibilities and job description for the Medical Social Worker (LCSW) position at Access TLC Health Care?
Access TLC Home Health Care is currently seeking dedicated and caring Medical Social Worker to join our Home Health team to visit patients in the San Bernardino County. The Medical Social Worker is responsible for assessing patient’s illness and environment relating to home health services, planning intervention based on evaluation findings and providing information and referral services for patients and families/caregivers.
All charting is done electronically, and we will provide you with an internet enabled iPad.
Position is per diem visits; visit rate is $70-75 per visit depending on the type of visit.
Access TLC is an accredited hospice and home health agency who provides unparalleled quality of service that allows patients to have a better quality of life. We have been serving Ventura, Santa Barbara, and Los Angeles counties for over 20 years.
Requirements include:
- Licensed as a licensed clinical social worker by the Board of Behavioral Sciences.
- Minimum of one year's experience in health care.
- Spanish speaking preferred.
- CPR Certification.
- Current California Driver's License and proof of insurance.

Pay: $35.00 - $75.00 per hour
