- Provide full-time skilled physical therapy services in patients’ homes
- Evaluate mobility, strength, balance, and functional limitations
- Develop and implement individualized treatment plans
- Educate patients and caregivers on home exercise programs and safe mobility techniques
- Monitor progress, document outcomes, and adjust treatment as needed
- Collaborate with an interdisciplinary care team to ensure coordinated, high-quality care
- Active Florida Physical Therapist (PT) license
- Degree from an accredited Physical Therapy program
- Valid driver’s license & reliable transportation
- Current CPR certification
- Strong communication, documentation, and time-management skills
- Home health experience (preferred)
