What you'll do as a Case Manager
- Provide case management services for up to 8 weeks to youth and families to prevent further problems in functioning and/or disruption of their living environment
- Provide culturally competent service delivery in the community
- Participate in innovative initiatives designed to engage communities in care and treatment

What you'll bring to the team
- Bachelor's degree in counseling, social work, or related field plus a minimum of one year of experience
- Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record. Local travel required
- Professionalism and a commitment to excellence in care
- Bilingual a plus
