Qualifications of Home Health Medical Social Worker(MSW):
- Have a Master's degree from a school accredited by the Council on Social Work Education
- Has a Bachelor's Degree in social work, psychology, sociology or other field related to social work
- One year of Home Health experience
- Ability to exercise initiative and independent judgment
- Must be a team player

Responsibilities:
- Understand and adhere to established agency policies and procedures
- Understands and promotes principles of continuous performance improvement
- Acts as an consultant for the agency
- Serves as a resource person to employees, patients, physicians and other allied healthcare providers
- Develops a cooperative relationship and communicates effectively and professionally with physicians
- Maintains current knowledge of Medicare Conditions of Participation for social work services.
- Reports any accident, incident, lost articles or unusual occurrence to the Director of Nursing/Assistant of Director of Nursing
- Plans and coordinates all social services within the Agency
- Work with family
