Job Requirements:
- Ensures that Accel Rehab achieves monthly budgeted census
- Assists referral sources in identifying patients with care needs that match the services provided by the Hospital
- Complete preliminary, standardized assessments on each new patient referral
- Educate the potential patient and family regarding services provided, admissions process, treatment process and projected outcome
- Communicate with appropriate nursing facility staff and ensure accommodations and staff are prepared for patient arrivals
- Coordinate patient transfers and necessary transfers and admissions paperwork
Minimum Qualifications:
- Must have one of the following licenses in Texas: Nurse, Therapist, or Social Worker
- A minimum of one (1) year of general clinical experience is preferred
- Ability to communicate with all levels of personnel
- Must have excellent assessment skills
