We are a Long Island–based, Nurse Practitioner–owned and led Medical House Call practice, established in 2018. We provide comprehensive in-home Primary and Urgent Care to patients of all ages, including those with complex and chronic conditions.

Responsibilities
- Provide Primary and Urgent Care to patients in their homes as an independent Advanced Practice Provider
- Care for patients of all ages with varying conditions, including complex disease management
- Perform comprehensive assessments, diagnose conditions, and develop individualized treatment plans
- Provide follow-up care to support continuity and optimal patient outcomes
- Prescribe medications, labs, imaging, DME, PT/OT, and wound care
- Utilize EMR and e-prescribing platforms efficiently
Qualifications
- Minimum 3 years or 3,600 hours of experience as a Family Nurse Practitioner
- Active New York State NP license
- Current ANCC or AANP certification
- Active NYS NPI and DEA
- Strong clinical, diagnostic, and therapeutic skills
- Comfortable practicing independently with patients of all ages and acuity levels
- Proficient with EMR, e-prescribing, and digital platforms
- Personal laptop and smartphone required
Schedule & Commitment
- 15–20 hours per week
- 2–3 weekday shifts (flexible hours based on provider availability)
- 3 weekend days per month
- Must have a registered vehicle and be willing to travel throughout Long Island
