Overview of the Job
- This position is for a Speech Language Pathologist to work as a Care Manager in our Home Health department.
Responsibilities:
- Perform evaluations and determine diagnosis, prognosis, and plan of care in consultation with physicians.
- Determine specific interventions, duration, and frequency needed to achieve anticipated goals and outcomes.
- Observe and record activities and findings in the clinical record and report to physicians and primary care managers.
- Instruct patients and caregivers on speech language improvement programs, adaptive equipment, and home programs.
- Select, apply, or modify skilled interventions to achieve the best outcome possible for patients.
- Assess patient response to interventions and perform reassessments as required.
- Document treatment provided and update care plans as needed.
- Complete all documentation within the required time frame, including reassessments, discharge summaries, and OASIS documents.
Salary : $80,000 - $120,000
