At Aegis Healthcare we are expanding our Atlas Medical Care team and are seeking a dynamic and experienced individual for the role of Bridge Patient Care Representative who shares our desire to make healthcare better for everyone.

COMPENSATION: $18 - $22/hr DOE
STATUS: Full-Time, W2 employee
SCHEDULE: M-F 8am to 5pm
LOCATION: East Valley (Office: 7227 E Baseline Rd. Ste 129, Mesa, AZ 85209)

Position Description and Responsibilities:
Aegis Homecare and Aegis Hospice is currently seeking compassionate a Patient Care Advocate that has a passion in providing outstanding services to the elderly through our Bridge Program in a senior living community setting. This position provides an added layer of peace of mind for families and bridges the gap between home care and hospice.
- Makes scheduled in-home patient visits.
- Educates families on the Aegis Health Care continuum
- Connects patients with outside resources
- Bridge the gap between home care and hospice for patients and families
- Provide companionship to seniors
SKILLS & QUALIFICATIONS:
- Must be at least eighteen (18) years of age
- Home Health / Hospice experience as a Certified Nursing Assistant (CNA), preferred
- Valid Driver's license, clean driving record and reliable automobile
- Proof of current automobile insurance (MVR checked)
- Criminal background check clearance/fingerprint clearance.
- Negative TB test
