Program Coordinator
Affinity Care seeks a designated Program Coordinator to join our supportive apartment team. The ideal candidate will be familiar with 245D standards and is passionate about providing person-centered support and empowering individuals with mental illness and intellectual and developmental disabilities.
Locations Available: Newport, MN
Shift: 8:30 am - 5:00 pm (Monday through Friday and every other weekend)
Essential Duties and Responsibilities:
- Manage a team of DSP's in the respective cluster group
- Participating in the development, refinement, and implementation of individualized program plans that will help individuals safely achieve their goals.
- Providing work direction, guidance, training, and feedback to staff to ensure the highest standards of service delivery are provided to the individuals.
- May need to assist with providing direct care to individuals such as bathing, toileting, hygiene, medication/treatment administration, providing escorts and mobile supports and behavior interventions.
- Contributing to the safety and security of the site, individuals served and co-workers
Must meet ONE of the qualifications to be a Designated Coordinator:
- A minimum of 50 hours of education and training related to human services and disabilities; and four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older.; OR
- Bachelor’s degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
- Associate’s degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
- A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; OR
- Must have a valid driver’s license and a good driving record.
- Must be willing to submit to a background study completed by the Department of Human Services.
- Must be willing to travel to different sites

Employees who travel will receive mileage reimbursement

Pay: $21.00 - $24.00 per hour
