The Community Health Worker (CHW) addresses health disparities and improves the health of older adults in suburban Cook County. This position builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as resource and system navigation, caregiver support, outreach, community education, informal counseling, social support and advocacy. Health equity is integral to the work of this position and their team.

Must be willing and able to travel within the greater Chicagoland area (primarily suburban cook county) regularly

Benefits
The starting salary range for this role is $38,000-$45,000 annually; based upon an individuals experience, education and certifications.
In addition to your annual salary AgeOptions makes a 5% contribution to your 403(b) whether your contribute or not!!
This is a full time role and includes ability to participate in our medical, dental and vision plans
We work in a hybrid environment and this role will have responsibilities in the office and in the field as well as the ability to work remotely regularly.

The Community Health Worker (CHW) addresses health disparities and improves the health of older adults in suburban Cook County. This position builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as resource and system navigation, caregiver support, outreach, community education, informal counseling, social support and advocacy. Health equity is integral to the work of this position and their team.
What are the Specifics (or at least some of them):
- Work directly with community members to screen for needs, provide health education, information and referrals to clients to address social drivers of health (SDOH)
- Support clients in the their follow-up, help build relationships with care providers and encouraging commitment to programs
- Conducts caregiver assessments using evidenced based assessment tools, assist caregivers and clients in accessing resources
- Implement outreach plans and special projects related to benefit assistance
- Facilitates evidence based health promotion programming virtually and in person
- Conducts outreach and education to individuals who are in need of health care and social services to increase utilization of the health care system
Required Experience
Who are we Looking for-- Someone with:
- At least 2 years of professional work experience working with diverse populations, community or faith based organizations in a social services or health care or health education related setting
- At east 1 year experience conduction screenings and/or assessments
- Experience speaking publicly to groups and individuals, facilitating training or workshops
- Associate degree in social sciences, human services, public health, counselling or related area if no job experience
- Community Health Worker training/accreditation attained or willingness to be attained while in role
- Strong, demonstrated project management skills and experience
- Bi-Lingual strongly desired
- Accountable, Self-starter with a passion for our Mission, Vision and Values
- Actively seeks to understand others perspectives, foster open dialogue, adapts communication style for audiences and is approachable and respectful
- Experience working in team environments that value and leverage diverse viewpoints.
- Capacity to manage multiple tasks simultaneously, effectively working both independently and as part of an inclusive team.
