Job ummary:
A Care Manager coordinates and manages clients’ care, ensuring access to necessary services and resources, while advocating for their well-being. They assess clients' needs, develop care plans, and facilitate communication between healthcare providers, clients, and their families. Care managers also monitor clients’ progress, educate clients and families, and help navigate the healthcare system. Role is primarily based on the Northshore of Boston but may require travel to other communitie

    Care Planning: Developing and implementing individualized care plans based on assessed
  • needs.
    Coordination of Services: Connecting clients with appropriate healthcare providers, community resources, and support

    Communication: Facilitating clear communication between clients, families, and healthcare profe
  • ssionals
    Monitoring and Evaluation: Tracking client progress, assessing the effectiveness of care plans, and adjustin

  • as needd.
    Advocacy: Representing the client's best interests and ensuring their voice is heard within the heal
  • thcare sysem.
    Education and Support: Providing information and support to clients and their families regarding their condition, treatment, and avai

  • able resouces.
    Documentation: Maintaining accurate and up-to-date client records and care plans, including billing/EMR entry within 24 hours of the activity (sa
  • me day prefeably).

    Community: Building relationships with community service providers and elder care professionals (attending mark
