Job ummary:
A Care Manager coordinates and manages clients’ care, ensuring access to necessary services and resources, while advocating for their well-being. They assess clients' needs, develop care plans, and facilitate communication between healthcare providers, clients, and their families. Care managers also monitor clients’ progress, educate clients and families, and help navigate the healthcare system. Role is primarily based on the Northshore of Boston but may require travel to other communitie
