The Neighborhood Team Leader (NTL) is responsible for using the Nursing and RAI processes to assess resident needs and coordinate the care on their assigned units. The NTL is also responsible for evaluating and supervising the other nurses and certified nursing assistants who directly perform resident care. The NTL is responsible for the management of the staff to carry out the total cares of the residents on a twenty-four-hour basis.
The Neighborhood Team Leader (NTL) role also includes the duties of a Nurse Assessment Coordinator (NAC). As defined in Appendix A of the RAI User’s Manual, the Nurse Assessment Coordinator (NAC) is “an individual licensed as a registered nurse by the State Board of Nursing and employed by a nursing facility and is responsible for coordinating and certifying completion of the resident assessment instrument.â€

Experience:

Duties/Responsibilities:
- Collaborates with residents, staff, and families/responsible parties.
- Develops, implements, and evaluates plan of care for each resident.
- Participates in interdisciplinary care conferences.
- Communicates specific resident needs to family, social services or appropriate personnel.
- Responsible to address staffing issues.
- Provide ongoing staff training/education and evaluations.
- Work closely with DON and team leaders, checking for problems/concerns, and resident care changes.
- Complete all assessments per individual assessment schedule.
- Performs daily rounds to supervise and monitor resident care.
- Participates and assists with admissions, discharges, and transfers of residents.
- Investigate resident falls, skin tears, and other incidents to create a plan to prevent.
- Provide assistance with survey process and plan of care.
- Attends Medicare/PPS meetings and other meetings as needed.
Required Skills/Abilities:
- Must be able to demonstrate leadership.
- Must demonstrate professionalism and positive approach toward others and work-related tasks.
- Demonstrates willingness to become familiar and adhere to state and federal regulations and infection control.
- Must have a genuine interest in geriatric nursing and a willingness to upgrade by attendance at workshops, seminars, and facility meetings and in-services.
Education and Experience:
- Must be a graduate of an accredited school of nursing.
- Must retain a current, unrestricted license in the state of Minnesota as an RN.
- Must retain a current CPR.
