ACCOA Direct Care Provider
Below are the minimum employment requirements
*Summary: Under the general direction of the ACCOA Board of Directors and the ACCOA policy guidelines, the service provider has the responsibility of maintaining an efficient performance of the duties assigned by the ACCOA Board of Directors and its Executive Director adhering to the personnel policies and procedures of the ACCOA Board of Directors.
Duties and Responsibilities:
· Assists individuals in dressing, bathing, and completing basic hygiene tasks.
· Oversee and required rehabilitation exercises
· Prepare meals that adhere to any special dietary needs/cooking
· Assist with cleaning and housework
· Laundry
· Grocery shopping/ post office
· Ensure that the clients are reaching their goals as outlined in the plan of care
· Properly document daily job duties
General Description:
· Must be neat and clean in appearance
· Must be friendly and polite
· Must be able to lift heavy objects
Qualifications:
· Two years of experience as a DCW
· CPR & first aid certification (Can obtain after hire)
· Successful completion of training programs provided by ACCOA
· Excellent written and verbal communication skills

· Must have reliable transportation with insurance
· No felony convictions (State Police Background Check)

Educational Background:
· High school diploma/GED
· Two years of experience as a DCW or care work setting
