FUNCTION
The Coordinator of Intensive Case Management is responsible for supervising the team of Intensive Case Managers, a subsection of
the Mental Health Program. The ICM team provides substance use screenings and assessments, mental health referrals, housing
referrals, treatment planning, advocacy, escorts, and coordination of services for homeless clients who are living with significant mental
health diagnoses. The Coordinator will also provide site supervision at the Ali’s Place (our Drop-In Center) and will provide clinical
trainings to agency staff, and assist the Mental Health team as needed.
TOP RESPONSIBILITIES
- Supervise a team of Intensive Case Managers under the SAMHSA contract and ensure program deliverables.
- Provide site supervision and mental health support at drop-in center such as crisis de-escalation, suicide assessments,
trainings, and other relevant needs.
- Maintain a caseload to provide ongoing intensive case management services (crisis counseling, treatment planning, housing
referrals, advocacy, escorts, and discharge planning) to.
- Coordinating with housing leadership and case managers to manage bed placements into AFC housing programs.
- Assist the Mental Health team as needed (Crisis debriefing, clinical coordination, trainings).
- Participate in weekly treatment planning meetings and provide clinical knowledge and expertise about mental health concerns
to direct care staff.
- Attend weekly staff meetings, care coordination meetings, provide individual weekly supervision to team members, supervise
interns when applicable.
EDUCATION REQUIREMENTS
[ X] Masters Degree
MSW degree from a CSWE accredited school of social work required; LMSW/LCSW preferred. SIFI preferred.
SKILL REQUIREMENTS
- TGNCNB Competency
- Proficient communication and writing skills.
- Knowledge of psychosocial needs of LGBTQ/homeless population.
- Basic assessment and/or interviewing.
- Computer and technology, data entry and documentation.
- Familiarity with trauma informed care and harm reduction.
PREFERRED QUALITIES
Must be knowledgeable in the skills of therapeutic engagement, substance use and abuse, as well as mental illness. Supervisory experience is preferred.
