SUMMARY:
The Floor Nurse provides support to providers and patients in accessing care by being a liaison and advocate for patient needs. The Floor Nurse will be available in the clinic to provide patient education, referral assistance, applications for patient assistant programs, assessment for urgent care requests, medication refills and phone calls. The Floor Nurse is also responsible for maintaining Abnormal Pap, Sexually Transmitted Infection (STI), and Elevated Lead tracking systems for site

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Patient Care Support
- Assists with urgent care requests and patient flow.
- Assists with phone calls related to medication refills, referrals, patient questions.
- Documents all care and patient interaction in the medical record according to policy.
- Follows approved protocols and standing orders for patient care.
- Completes applications for Patient Assistance Programs.
- Serves as a resource for MA’s when RN Clinical Coordinator not available.
- Provides assistance during emergency situations.
- Maintains ongoing communication with mid level providers and physicians to assure quality of care.
- Initiates and/or participates in quality assurance and continuous quality improvement.
- Communicates normal and abnormal lab results to patients including education and treatment plan as outlined by the medical provider
Employee Health
- Performs employee health screenings and collects data.
Educational Responsibilities
- Serves as an advocate and provides instructions and counseling to patients and their families including planning, implementing and teaching/education.
- Provides patient education material and instruction, including but limited to: breastfeeding, baby care, diabetes education, asthma management, and family planning.
Tracking Responsibilities
- Identifies abnormal vs normal pap results.
- Consults with Advanced Practice Nurse or MD as needed.
- Informs patient of abnormal pap result, treatment plan within a timely manner.
- Assists in scheduling colposcopy appointments, educate patient regarding colposcopy procedure.
- Notifies Chicago Department of Public Health of positive Gonorrhea and Chlamydia within five days of diagnosis or treatment.
- Develops and implements efficient tracking systems for STI, Pap and abnormal lead.
- Attends appropriate clinical meetings related to patient care.
- Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG guidelines, other State and Federal laws).
- Performs other duties as designated by supervisor.
This position requires compliance with all of Alivio’s written standards, including its Standards of Conduct, Joint Commission standards, all policies and procedures and Corporate Compliance requirements. Compliance will be considered as part of the regular performance evaluation.

Manages and supervises staff listed below, including hiring, training, evaluating and managing performance to meet departmental efficiency and performance measures in keeping with the Union Collective Bargaining Agreement (CBA), Personnel Policies and Procedures and prevailing employment laws as appropriate

QUALIFICATIONS:
Education:
- Graduate of Accredited School of Nursing
Certification/Licensure:
- Current LPN or RN Illinois state license
Experience:
- One-year experience in ambulatory care preferred
Special Training:
- CPR Certification
Demonstrated Competencies:
- Must be bilingual – English/Spanish
