Position Overview:
We are seeking a skilled and compassionate Registered Nurse (RN) to join our hospice care team. The ideal candidate is a strong communicator who thrives in an autonomous role and is passionate about providing holistic, patient-centered care.
Key Responsibilities:
- Conduct initial and ongoing patient assessments to determine physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with physicians and the interdisciplinary team.
- Provide direct skilled nursing care, pain and symptom management, and emotional support to patients and families.
- Educate family members on end-of-life care, medications, and comfort measures.
- Document all visits accurately and timely in accordance with agency policies and regulatory standards.
- Participate in on-call rotation and interdisciplinary meetings.
- Active Registered Nurse (RN) license in Pennsylvania - required
- Minimum 2 year of professional hospice experience - strongly preferred
- Demonstrated ability to perform thorough clinical assessments, prioritize effectively, and manage complex caseloads with autonomy
- Exceptional communication, critical thinking, and documentation skills
- Valid driver’s license, reliable transportation, and willingness to travel to patient homes as needed
