Overview
We are seeking a compassionate and dedicated Registered Nurse to join our In-Home Care team. The ideal candidate will possess a strong commitment to patient care. This role requires the ability to work independently. The Registered Nurse will play a lead role in patient assessment before services are rendered by the CNA or PCA.
Duties
- Conduct thorough patient assessments, including vital signs and medical history reviews.
- Maintain accurate medical records and documentation for each patient.
- Updating database records
- Collaborate with case managers and physicians orders to develop and implement individualized care plans.
- Participate in nursing assignments as needed, adapting quickly to new environments. Must be available and willing to visit patient in their home.
Qualifications
- Current Registered Nurse (RN) license in good standing.
- Proficiency in patient assessment techniques and vital sign monitoring.
- Familiarity with medical records management systems.
- Strong communication skills and the ability to work collaboratively within a team.
- Compassionate demeanor with a focus on patient-centered care.
- Ability to handle high-stress situations effectively while maintaining professionalism.
