Evaluate and Admit clients for Agency Services.
Completes and submits documentation within agency timeliness policies that is complete, legible, and clearly reflects care given.
Obtain Required Auth’s for services via Portal, following thru to completion. Communicates Auth information and SOC with other team members to assist in coordinating care.
Position works closely with DHS RN’s, Families, Caregivers and other community resources on behalf of client.
Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing
Applies nursing principles and techniques in the treatment of patients. Provides limited nursing care, emotional support and education to patients and families. Encourages patients and family members to participate in the patient’s plan of care to meet established goals.
Prefer Registered Nurse with Home Health and Medicaid Experience.
