Job Overview:
The Behavioral Health Treatment Coordinator for the CSAT MAI Program will focus on providing compassionate care and coordinating navigation and linkage to Substance Use Disorder (SUD) and Co-occurring Mental Health Disorder (COD) treatment for racial/ethnic minorities, LGBTQI populations, individuals at risk for or living with HIV, the aging population, and individuals facing housing instability. The Coordinator will ensure clients receive comprehensive assessments, develop individualized care plans, and are linked to safe spaces, housing resources, tobacco cessation programs, and crisis support systems. A key part of this role includes addressing suicide ideation through QPR (Question, Persuade, Refer) and other intervention strategies. Collaborating with Recovery Coaches, Health Navigators, Specialists, Health Educators, and community partners, the Coordinator will expand referral systems of care, including linkages to recovery housing, and work to ensure whole-person, trauma-informed treatment that addresses the full spectrum of client needs.
Key Responsibilities:
1. Client Navigation, Linkage, and Retention to Care:
- Serve as the primary navigator for clients, ensuring they are linked to SUD and COD treatment, detox services, HIV care, housing support, recovery housing, and tobacco cessation programs.
- Collaborate with Recovery Coaches, Health Navigators, and Specialists to develop individualized navigation plans that address each client’s unique needs, with a focus on retention in care.
- Work with community organizations to identify and expand safe spaces, support systems, and trauma-informed care for victims of intimate partner violence (IPV), survivors of childhood sexual abuse, and individuals experiencing housing instability.
2. Overseeing the Implementation of SUD and COD Assessments:
- Conduct client assessments using tools such as the Client Diagnostic Questionnaire (CDQ), Modified Simple Screening Instrument for Substance Abuse (MSSI-SA), Drug Abuse Screening Test (DAST), and Adverse Childhood Experiences (ACE) to develop comprehensive care plans for individuals facing SUD, mental health, and trauma challenges.
- Coordinate care based on assessment findings, ensuring clients have access to appropriate mental health and substance use services, especially those living with or at risk for HIV.
- Ensure that assessments are trauma-informed and take into consideration the unique needs of individuals experiencing housing instability and other social determinants of health.
3. Addressing Suicide Ideation and Crisis Support:
- Implement QPR (Question, Persuade, Refer) strategies to address suicide ideation among clients, ensuring timely referrals to crisis intervention services and mental health professionals.
- Provide compassionate support to clients struggling with suicide ideation, anxiety, and other mental health challenges, ensuring they receive the appropriate crisis and mental health care.
- Ensure all mental health interventions are trauma-informed, recognizing the long-term impacts of IPV, childhood sexual abuse, and housing instability on mental health and recovery.
4. Therapeutic Modalities and Evidence-Based Interventions:
- Coordinate evidence-based therapeutic interventions, including Motivational Interviewing (MI), Cognitive Behavioral Therapy with Behavioral Activation (CBT-BA), and Contingency Management (CM), ensuring that treatment is culturally responsive to the needs of BIPOC, LGBTQI , and housing-insecure populations.
- Facilitate group and individual support sessions that help clients engage in their recovery and build long-term strategies for SUD and COD management.
5. Collaboration with Recovery Coaches, Health Navigators, and Specialists:
- Coordinate closely with Recovery Coaches to guide clients through their recovery journey, addressing both their SUD treatment and overall health needs.
- Collaborate with Health Navigators and Specialists to expand referral networks and ensure clients can access primary care, HIV care, mental health services, housing assistance, recovery housing, and tobacco cessation programs that address social determinants of health.
- Provide training and guidance to Health Educators on delivering educational programs focused on substance use, mental health, HIV prevention, tobacco cessation, and trauma recovery.
6. Expanding Referral Systems of Care and Community Partnerships:
- Build and strengthen relationships with community partners to expand the referral system and enhance the quality of care for clients, including housing programs, legal aid services, employment agencies, recovery housing, and other critical support systems.
- Participate in advocacy meetings and community events to promote SUD services, advocate for expanded access to care, and raise awareness about the needs of racial/ethnic minorities, LGBTQI populations, individuals facing housing instability, and those living with or at risk for HIV.
7. Client Retention and Engagement in Care:
- Develop strategies to retain clients in SUD, COD, and HIV care, ensuring they remain engaged with recovery programs and receive continued support.
- Track client progress through regular follow-ups, adjusting care plans as necessary to meet clients’ evolving needs and improve retention in care.
- Utilize EMR and other customized data systems to track outcomes and provide reports on client progress and program effectiveness.
Qualifications:
- Education: Master’s Degree in Social Work, Counseling, Psychology, Behavioral Health, or a related field (preferred); National Certified Counselor (NCC) or equivalent certification required; Certified Alcohol and Drug Counselor (CADC) required.
- Experience:
- At least 3-5 years of experience in SUD and COD treatment, including work with survivors of intimate partner violence, childhood sexual abuse, and individuals experiencing housing instability.
- Experience working with BIPOC, LGBTQI , and aging populations at risk for or living with HIV.
- Experience conducting SUD and mental health assessments using tools such as CDQ, MSSI-SA, DAST, and ACE.
- Proven experience expanding referral systems, building community partnerships, and advocating for underserved populations.
Skills and Knowledge:
- Strong knowledge of SUD and COD treatment modalities, including MI, CBT-BA, and CM, and experience in delivering trauma-informed care to clients facing multiple challenges.
- Ability to collaborate with a multidisciplinary team, including Recovery Coaches, Health Navigators, Specialists, and Health Educators, to ensure clients receive whole-person, compassionate care.
- Proficiency in using RedCap, EMR, Bontera Solutions or similar data management systems to track client outcomes and ensure program compliance.
