Known for our culture of excellence, professionalism, and empathy, we are expanding our elite nursing services and searching for a Registered Nurse Care Manager to Join our team.
As an RNCM, you’ll serve as the "quarterback" of care, coordinating every aspect of the patient experience with precision, empathy, and attention to detail. This role is ideal for someone who thrives on making a real impact in the lives of patients and their families.
About the Role :
Act as the primary point of contact for patients and their families, ensuring their needs are met with compassion and respect.
Develop and oversee individualized care plans in collaboration with the Director of Operations, caregivers, and medical providers.
Foster trust through clear, consistent, and compassionate communication with clients, families, and care teams.
Monitor and evaluate patient progress, adapting care plans as needed to achieve optimal outcomes.
Maintain detailed records to support seamless care coordination and compliance with regulations.
This is a Monday-Friday 9AM-5PM role, along with occasional on-call responsibilities. Approximately 70% of your time will be spent in the field, visiting client’s homes.

Qualifications :
Bachelor’s in Nursing (Master’s preferred).
Active New York State nursing license.
5 years of nursing experience.
Exceptional organizational, problem-solving, and communication skills.
