Major Tasks, Key Responsibilities and Key Accountabilities

— 1 Completes an initial assessment of patient and family to determine home care needs.

— 2 Regularly reevaluates patient nursing needs

— 3 Initiates the plan of care and makes necessary revisions as patient status and needs change

— 4 Uses health assessment data to determine nursing diagnosis

— 5 Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. 

— 6 Includes the patient and the family in the planning process

— 7 Initiates appropriate preventive and rehabilitative nursing procedures. 

-- 8 Administers medications and treatments as prescribed by the physician

Job Overview and Requirement

Job Type: Registered Nurse-Home Health

Qualifications:

• Graduate of an accredited school of nursing. One to Two years of recent acute care experience in an institutional setting
  
• Current RN license in the State of Colorado
• CPR Certified
• Bachelor's Degree, with one year of home health care experience preferred.
  
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills, nursing skills, etc.
• Reliable transportation and license to drive.

Home Health – Per Visit

o RN Evaluation / OASIS Recertification / OASIS DC / OASIS Resumption
       -$80
o Routine Visit / Reevaluation / PT Discharge
       -Rate determined by years licensed up to 10 years.
                  -Starting Rate: $55.00
