Objective:
Supports the operations, programs and activities for services provided under the NYSDOH Home and Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury (TBI) and/or Nursing Home Transition and Diversion (NHTD) Waiver.
The Service Coordinator advocates and coordinates services for consumers on the TBI and/or NHTD Waiver Program(s). The Service Coordinator (SC) ensures all necessary waiver services are implemented and maintained in compliance with all relevant regulations and policies, and in doing so will meet the following obligations: Supports the operations, programs and activities for services provided under the NYSDOH Home and Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury (TBI) and/or Nursing Home Transition and Diversion (NHTD) Waiver. The Service Coordinator advocates and coordinates services for consumers on the TBI and/or NHTD Waiver Program(s). The Service Coordinator (SC) ensures all necessary waiver services are implemented and maintained in compliance with all relevant regulations and policies, and in doing so will meet the following obligations:
Education & Experience
- The provider shall have at a minimum, one (1) year of experience providing Service Coordination in human services and Master’s degree.
OR
- Be an individual with a Bachelor’s degree and two (3) years of experience providing Service Coordination in human services.
Skills & Abilities:
- Demonstrate the correct use of spelling and grammar.
- Ability to read, write, speak and listen. Ability to effectively present information to co-workers and supporters.
- Ability to perform basic math.
