Title: Transitional Living Coach
Classification: Full-time
Reports To: Director of Program and Performance
Position Overview:
Under the general supervision of the Program Support Aid and Program Service Coordinator, the Transitional
Living Coach will work closely with Members and program staff to assist with the delivery of services to members of Almost Home. Responsibilities include monitoring the safety and well-being of up to 15 member and 25, providing 1:1 assistance and support to Members and modeling appropriate interactions. The
Transitional Living Coach will work to ensure full implementation of and compliance with the program requirements.
Core Functions
o Coaches and encourages members to make positive behavioral changes, providing feedback and appropriate alternatives.
o Ensures service delivery of daily program activities including meals and housekeeping.
o Assists in kitchen – including completing cleaning, organizing, cooking, and serving meals as needed.
o Monitors resident activities ensuring Member and child safety.
o Works closely with members and program staff on achieving goals identified in service plans.
o Model appropriate interactions and provide crisis intervention as needed.
o Assists and monitors members with chores, housekeeping, schedules, appointments, and other program requirements.
o Tracks member performance on accountability system and maintains weekly checklist.
o Distributes medication and accurately completes summary logs.
o Completes all program related documentation ensuring issues are clearly and effectively communicated – including incident reports, maintenance requests.
o Assist with member inquiry, admission and exit procedures.
o Maintain member confidentiality, professional boundaries as well as safety and ethical standards.
o Uses Motivational Interviewing, solution-focused, and trauma-informed approaches in all member interactions.
o Participates in & completes in-service training requirements with a minimum of 40-hours annually.
o Check work email and Teams at the beginning of each scheduled shift.
o Provides basic childcare supports as needed.

Qualifications:
o High School Diploma or GED.
o Proficient in use of technology including email and Microsoft Office software.
o Excellent written, verbal, and interpersonal communication skills.
o Belief in team approach and proactive in sharing information with others.
o High level of organization, initiative, attention to detail and follow-through
o Experience working with hard to reach and at-risk individuals and families.
o Ability to exhibit emotional control, patience, and persistence in stressful situations.
o Cultural competency and ability to establish positive rapport with members.
o Able to manage confidential information with discretion and tact.
o Valid driver’s license and proof of insurance; must obtain Chauffer’s license within 30-days of employment.
o Must pass a first aid and CPR course and receive certification in administering medicines within 30 days of hire.
Preferred Qualifications:
o Associates or bachelor’s degree in a social service field
o Minimum of one-year work experience in a residential setting-Required.
o At least one year of experience working with women and children desired-Required.
Physical Requirements:
o Walking, standing, bending, stooping, kneeling, and lifting up to 40-pounds.

Job Type: Full-time
