Job Summary
The Health Care Coordinator provides support to program participants with regard to healthcare, medications, and medical appointments. The Health Care Coordinator interacts with program participants and must be familiar with the Guiding Values issued by the Developmental Disabilities Administration (DDA). At all times, the Health Care Supervisor will seek to act in accord with the principles and values expressed in the guidelines and uphold the agency mission.
Day-to-Day

Participant Health Oversight:
- Obtain, review, and maintain participant and guardians consents
- Regularly communicate with assigned participants' healthcare and support teams
- Schedule and attend medical, mental health, dental, and specialist appointments
- Obtain health records, coordinate alternate staff for conflicts, and communicate updates to staff and guardians
- Verify immunizations and vaccinations based on physician and public health recommendations
- Monitor nutrition needs and assist with meal planning

Healthcare Records Management:
- Document appointments and maintain electronic and/or hard copy files
- Manage immunization, vaccination, and interval-based records (physicals, dental cleanings, psychiatric appointments)

Nurse Delegation and Medication Oversight:
- Order and fill new prescriptions and medication refills
- Provide oversight of electronic and/or written monthly medication records
- Review and coordinate nurse delegation protocols and training records
- Maintain documentation for psychoactive medications, including treatment plans and informed consent

Training Development:

Assist in developing and facilitating healthcare-related training for both new and experienced employees

Who we are looking for
- Basic to intermediate knowledge of nutrition, healthcare, and developmental disabilities diagnoses
- Capability to compile, categorize, calculate, audit, or verify information and data
- Intermediate to advanced understanding of principles and processes for providing supported living and personal care supports
- Identify developmental, educational, or training needs; contribute to the development of training programs; facilitate teaching or instruction
- Basic knowledge of performance management principles and personnel supervision
Job Requirements
- Minimum age requirement: At least 18 years old
- Educational Background: Associate degree or equivalent or one year of experience in social services and one year performing related duties
- Possession of a valid Washington State Driver’s license, current vehicle insurance, and the ability to be covered on Alpha Supported Living Services’ automobile insurance plan based on driving record
- Clearance from an authorized state agency through a background inquiry
Starting Wage:
All Health Care Coordinators will start at $26.50/hr
Wage Scale:
$26.50 - $32.10
Benefits:
Benefits for Full-Time Employees (30 hours or more a week) include Medical, Dental, Vision, 403(b) (non-profit version of a 401(k)) with company match, Life Insurance, Tuition Savings Program, and Tuition Reimbursement. Employees working 34 or more hours a week are eligible for PTO accrual.
