Job Description:
AgeWell Medical Associates is a primary care practice that specializes in providing comprehensive medical care to seniors aged 60 and above. AgeWell Medical Associates consists of two physicians, four APP’s and a psychologist providing on site geriatric behavioral health support. The practice caters to seniors with varying health conditions, ranging from those who are well to those with multiple chronic conditions. Patient-centered care is the practice's north star, with the goal of helping patients achieve their health goals.
Our practice is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.
The Community: Located in Colorado Springs, CO. Close to great biking, hiking and various entertainment venues. This community is touted as one of the best destinations in Colorado to raise a family and is recognized nationally by Forbes as one of the top places to move in the country.
The Position:
• Ambulatory primary care for older adults (60 and up)
• Innovative patient-centered care
• Collaborative open workspace
• No hospital or facility responsibility
• Office hours, Monday through Friday, 8:00 am to 4:30 pm
• 7 to 10 patients per day
• Dedicated MA for each provider
• One RN who manages transitions of care (hospital, ED, facilities)
• Night and weekend call-shared with other physicians (minimal calls)
Qualifications (Education/Experience/Knowledge/Skills/Abilities):
- MD or DO degree from fully-accredited medical or osteopathic school of medicine
- Residency trained in family medicine with adult medicine experience.
- Excellent communication and inter-personal skills

Benefits:
- Compensation and Benefits Package for a full-time physician
- The incoming physician will join AgeWell as an employee. The physician will have an opportunity to become a partner in the practice after one year.
- The practice is offering a base salary of $250,000 for the first year.
- 5 days CME up to $2,500 per year.
- PTO 4 weeks.
