Key Responsibilities:
- Patient Care: Provide direct nursing care to patients in their homes, ensuring the implementation of care plans in line with physicians' orders.
- Assessment & Documentation: Regularly monitor vital signs, track medical conditions, document changes, and maintain accurate health records.
- Medication Management: Monitor patients’ medication usage and report results, ensuring proper administration and dosage.
- Collaboration: Work closely with the interdisciplinary team, patients, and their families to assess, implement, and evaluate care plans.
- Personal Care: Assist with emotional, personal, and medical support for patients, providing holistic care to meet their needs.
- Care Coordination: Evaluate patient behavior, diagnose conditions, and consult with healthcare professionals to improve patient outcomes.
- Work in patient homes, often in varying environmental conditions (including possible exposure to bodily fluids and infectious diseases).
- Ability to travel locally to patient homes and manage flexible work hours.
- Occasional emergency PRN calls and exposure to adverse weather.
- Reasonable accommodations may be made for individuals with disabilities.
- Education: Graduate of an accredited school of vocational nursing
- License/Certifications:
- Current state LVN License.
- BLS Certification (required).
- Valid Driver’s License.
- Experience: Minimum of 2 years as an LVN in Clinical, Hospice, or Home Health settings (preferred).
