What are the responsibilities and job description for the PUBLIC HEALTH NURSE (PHN) position at Amergis?
The Public / Community Health Nurse (PHN) is responsible forthe mental and physical well-being of the community by ensuring the publicshealth through health promotion, disease prevention, treatment, and education.They can work in a variety of institutions such as the community, hospitals,clinics, or long-term care facilities while collaborating with stateregulators, doctors, and other healthcare professionals to keep the communitysafe and healthy.
Essential Duties and Responsibilities :
- Assesses health care trends in communities
- Collaborates with emergency response planning and trainingwhen applicable
- Works with public health officials to help communitiesaccess and receive care
- Provides immunizations and health screenings to all populationsin the community
- Provides patient and community education
- Monitors and delivers high-quality patient care
- Records and analyzes medical data in various communities
- Evaluates the health of patients and develops treatmentplans in collaboration with medical providers and multidisciplinary teammembers
- Participates in the evaluation and documentation of patientsand care
- Assists in developing, evaluating, and implementingprofessional development education
- Refers patients to other providers as needed
- Manages budgets of public health facilities if applicable
- Reviews the work of assigned team members for thoroughness,soundness of recommendations, and compliance with the standards, policies,procedures, and regulations of the CDC as well as the County Department ofPublic Health
- Performs other duties as assigned / necessary
