Job Description:
- Assist new residents and their family in the initial adjustment phase, as well as in the ongoing transition to institutional living
- Provide information to the resident, and/or family member, on residents' rights and responsibilities
Qualifications:
· Must have NY BSW or MSW
· Must have experience as a Staff Social Worker
· Must have long term care/nursing home experience
· Must have excellent outreach, assessment, written and verbal communication skills.
