Responsibilities

As a Medical Social Worker at American-Mercy Home Care, you will coordinate care and foster a supportive environment that enhances patients' social, emotional, functional, and physical health.

You will establish care plans in line with the physician's treatment strategy, ensuring personalized care for every patient.

In times of crisis, you will provide much-needed intervention and understanding as a pillar of support for patients and their families.

Our patients' lives are influenced by various social, emotional, and economic factors. With your insight, you will help physicians and caregivers understand and address these elements impacting medical progress.

You will connect patients with vital community resources, ensuring they have access to everything needed to thrive.

Qualifications

• Master's degree from an accredited school approved by the Council on Social Work Education
• Current certification through Ohio Board of Social Work
• One year of social work in a healthcare setting is preferred/home care experience will be advantageous
• Must obtain and maintain CPR certification

Pay Range
$22.91 - $31.50 /hour
