POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans
MINIMUM REQUIRED QUALIFICATIONS – WA RN License; BLS; BSN or ADN; 2 years case management; Prior travel experience; Recent Epic experience, clear communication skills, discharge planning experience, patient progression experience, ability to collaborate with other disciplines.
PREFERRED QUALIFICATIONS – Healthcare financial and reimbursement knowledge; Leadership and emergency services experience
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 10 hour shifts (0800a-0630p) with every other weekends. 40hrs/wk.
