Description
POSITION: Patient Care Unit Coordinator
REPORTS TO: Vice President of Caldwell Service Area
FLSA STATUS: o exempt o non-exempt
POSITION SUMMARY:
Directly supervise patient care unit and assist the director of nursing services in coordinating clinical services to all hospice patients
EDUCATIONAL QUALIFICATIONS:
- Current NC license as a registered nurse
- Bachelor of Science degree in nursing required or associate with ten or more years hospice and proven leadership
- Minimum of 2-3 years nursing experience required
- CPR certification
- Supervisory experience of two years or more
- Active patient contact or completion of a nursing refresher course within the past 3 years

- Knowledge of and commitment to the hospice philosophy of care
- Ability to work on an interdisciplinary team
- Ability to work a flexible schedule
- Ability to work effectively with others in a supervisory role
- Willingness to work with patients/families regardless of race, color, creed, age or sexual orientation. Must respect their beliefs and philosophy with an attitude of openness and receptiveness
TRAINING-Initial And Ongoing:
- Complete Hospice volunteer/staff training program
- Complete Hospice orientation program
- Obtain 12 hours of continuing education per year, including mandatory safety in-service
- Attend minimum of six staff meetings per year
- Maintain CPR certification
Requirements
DUTIES AND RESPONSIBILITIES:
- Assist in orienting new team members, as requested
- Participate in quality improvement program
- Adhere to strict confidentiality
- Adhere to safety policy and practices
- Complete employee time record accurately per policy
- Cooperate with all government agencies and outside auditors
- Assist with updating policies and procedures, as appropriate
- Complete annual TB screening
- Assure all license, certifications, continuing education information and any other required documentation on patient care unit nursing staff is submitted to human resources and kept current
- Maintain up-to-date documentation required and specified in Hospice policy and procedure
- Perform other duties assigned by supervisor
- Perform the duties of a primary nurse, patient care unit nurse, or admitting nurse, as needed
- Assist in performing the duties of the director of nursing services in her absence or as needed (See “director of nursing services job description”)
- Maintain staff schedule for patient care unit assuring 24 hour nursing coverage
- Work with senior vice president of nursing services and vice president of support services to assure continuity of service delivery and patient care
- Participate as member of Leadership Team
- Participate as member of Safety Committee
- Monitor pharmacy compliance on the patient care unit
- Follow all infection control procedures
- Interview, hire and supervise all patient care unit nursing staff, complete all performance evaluations and clinical competencies
- Compile employee infection reports
- Act as TB resource person for patient care unit staff
- Participate in job recruiting
- Audit patient care unit patient medical records of deceased and discharged patients
- Make daily patient care unit visits to evaluate care provided to patients
- Assure all patient care unit nursing staff adhere to personnel policies
- Responsible to assist with entering hospice and palliative care referrals in computer program
- Rotate administrative backup call
PHYSICAL QUALIFICATIONS:
- Speech and hearing skills necessary for telephone interaction and face to face contact
- Visual acuity to read chart and physician’s orders
- Ability to stand and walk for long periods of time
- Manual dexterity to establish IV sites, manipulate tubing, and other patient care supplies
- With assistance, ability to lift and support an average of 150 pounds
- Ability to sit as required to perform duties
- Ability to pull/push, and bend frequently
- Ability to squat, kneel, climb, and balance
SPECIAL REQUIREMENTS:
- Must have an automobile with required insurance coverage and a states driver’s license
Salary : $38,800 - $49,200
