Bridge Coordinator

Job Description:

• The Bridge Program is a set of eyes and ears for the family, providers, and facility staff. The Bridge Program provides support to our elderly and most vulnerable patients and their families navigating outside resources.
• The Bridge is there to bridge the gap between Home Health, Hospice, and any other service in-between.
• Bridge provides continuity of care.
• Home Health and Hospice providers take care of disease and symptom management.
• Bridge helps take care of the emotional and social needs of the patients as well as assisting with finding resources in the community needed or desired by the patient and their families.
• The Bridge Representative assists with communicating any changes or concerns to their care team.
• The Bridge Representative will visit each of their patients at least one time per month and will be checking in via phone in addition to visits as needed.
• The Representative will address any needs, wants, or concerns by doing research for community resources and relaying information to patients and/or families.
• The Bridge Rep will be expected to have professional and respectful communication skills while working within the field.
• Will also keep an open line of communication with the Bridge Manager.
• The Representative must be well organized and have good time management skills.
• The Bridge Rep must be self-sufficient and be able to appropriately schedule visits.
• Previous experience with working in the healthcare field preferred.

Location: Wasilla, AK

Pay: $18-$20 an hour

Licenses/Certifications:

• Valid Driver's License and Auto Insurance
• Must complete required competencies to validate and maintain competency

Physical Demands:

• Occasionally (up to 33%) - Standing, Bending, Kneeling, Twisting, Lifting up to 50 lbs., Carrying up to 50 lbs., Driving.
• Frequently (34-66%) - Walking
• Continuously (67-100%) - Sitting

Education/Experience:

• High School Diploma or Equivalent

Working Environment:

• Works in the field and occasionally in the Agency office.

Job Relationships:

• Supervised by Administrator, Bridge Manager.

Risk Exposure:

• Low risk

Salary : $18 - $20
