•Compensation: $19/hour
•Status: 32 hours/week (opportunity to increase to 40 hours with census), W2 employee
•Schedule: Monday-Friday
•Location: Anchorage, AK

Position Responsibilities
As a Bridge Care Coordinator, you'll provide exceptional care and support for our aging population through our Bridge Program. Your role will:
•Conduct scheduled in-home patient visits.
•Educate families about the Ancora Health Care continuum.
•Connect patients with external resources.
•Bridge the gap between home care and hospice for patients and families.
•Provide companionship and peace of mind to seniors.
Skills & Qualifications
•At least 18 years old.
•Healthcare experience required.
•Highly organized with strong computer skills and ability to work independently.
•Valid driver's license, clean driving record, and reliable vehicle.
•Proof of current auto insurance (MVR check required).
•Negative TB test.
