Behavioral Health Outpatient Clinician - Augusta

Per Diem

Augusta Office, Augusta, ME, US

Title: Outpatient Clinician, Augusta area Assisted Living Facilities- On Site

What You’ll Do:

• Assesses and identify the client’s holistic health care needs/concerns as part of a family and community.
• Provide psychosocial assessments for clients.
• Develop individualized treatment plans for youth, adults, and families.
• Implement patient care plan activities based on treatment goals towards independent living for the patient/family.
• Serve as resource for staff education & training, participate in case conferences as needed.
  
  

What You’ll Bring:

• Master’s degree from a school of social work accredited by the council on social work education.
• Maine Board of Social Work Licensure: LCPC, LCPC-C, LCSW or LMSW-CC
• Current CPR card required.
• One or more years of experience as a master’s level social worker in a health care setting.
• A valid Drivers License and reliable car with insurance.
