PRIMARY PURPOSE OF POSITION: Responsible for providing patient and family centered hospice care to individuals with a life limiting illness. The Hospice LPN works with the interdisciplinary team (IDT) to provide holistic care for hospice patients and their families. Nursing intervention is under the supervision of an RN and in conjunction with the physician and written plan of care.
EDUCATION: Graduate of an accredited NLN School for Practical Nursing. Satisfactory completion of approved course in pharmacology.
EXPERIENCE: 1 to 2 years of recent nursing experience required. Community health or hospice nursing experience desirable.
LICENSURE: Current LPN license in the State of Maine & CPR Certification.
JOB LOCATION: This position is based out of our Norway office. This position entails exposure to infectious diseases and considerable physical requirements including lifting and transferring patients.
SCHEDULE: Monday-Friday , rotating weekends, rotating holidays, on call requirements.
EQUIPMENT USE: Includes but is not limited to blood pressure set, glucose monitor machine (as needed), hoyer lift, basic patient care supplies, dressing supplies, IV supplies, oxygen equipment/supplies, nebulizer machine, suction set up, and venipuncture supplies.
OTHER JOB REQUIREMENTS:
- Must have reliable transportation.
- Must have a current Maine Driver’s license.
- Must provide proof of automobile insurance.
- Must submit current LPN license every 2 years to Human Resources Dept. on or before expiration date.
- Must re-certify in CPR every two years.
- Must submit accurate time and attendance records according to Agency protocol.
Supervised By: Hospice House Clinical Manager
Direct Patient Care: Act as patient advocate. Provide high-quality care to hospice patients and their families, requiring nursing services. All activities are done under the direction/supervision of a Registered Nurse.
- Give medication as prescribed by the medical provider, and within licensure
- Take and record patient vital signs as needed
- Provide basic wound care including cleaning and bandaging as ordered
- Provide immunizations as ordered
- Provide patient and family education
- Obtain admission consents
- Take patient medical histories
- Manage IVs
- Ensure patients and their families understand discharge instructions
- Supervise CNAs (Certified Nursing Assistants)
- Assist with general cleaning
- Provide patient snacks/meals as needed
- Observe patients’ for reactions to medications
- Assist with daily needs such as bathing and dressing
- Observe patients’ skin for potential bed sores and provide optimal positioning
- Follow prescribed plan of care developed by the primary RN.
- Provide supportive physical, emotional and spiritual care.
