ESSENTIAL DUTIES: (Individual duties, assignments, and responsibilities required of the position.) |
- Communicate with health care professionals and families regarding clients’ health status.
- Conduct appropriate health-related in-services as required.
- Communicate with pharmacy regarding medication changes.
- Actively participate in the interdisciplinary team process assisting in the formulation of care plans and treatment objectives.
- Coordinates and conducts assessments such as the HRST assessment, nursing assessment, and other related nursing assessments required by agency policy and the Department of Human Services Rule 116 of each program participant’s medical needs to establish appropriate priorities.
- Coordinates and conducts instruction to program staff on methods and techniques in delivering medical care and treatment as specified in the Personal Plan and/or Implementation Strategy.
- Ensure that each program participant’s progress in response to medical care rendered documented.
- Works with other nursing staff, Clinical Director, Assistant Residential Director and the Medical Service Coordinator in recommending an approval or denial of a program participant’s return to an Anixter Residential Program when discharge from an inpatient hospitalization are planned by attending hospital staffing or visiting the program participant.
- Collaborates with other members of the interdisciplinary team in staffing when medical condition affects daily programming as identified in Personal Plan and or Implementation Strategy.
- Coordinates and provides classroom and on-site training to direct support staff on the medical issues, interventions, techniques, medication administration required by each program participant.
- Completes all agency training requirements and secures additional information, education, and training as is appropriate to remain abreast of new knowledge, technologies, and methodologies including but not limited to IL DHS Rule 116.
- Ensure adequate understanding of concepts and principles upon which work responsibilities are base and acquires requisite skills to perform job duties effectively and ensure agency services are provided consistently within its mission and objectives.
- Facilitates communication, teamwork and cooperation by reviewing quarterly program statistics in order to ensure effective service implementation and the identification of additional needs.
- Lead weekly medication review in collaboration with the Quality Department.
- Reports all safety hazards and makes suggestions to eliminate, reduce, and manage safety risk and problems.
- Oversees the destruction of medications in accordance with agency, state, and federal rules, regulations, and laws.
- Stays flexible to accommodate the specialized needs of the department including medication passes when necessary, scheduling medication administration training classes and authorization sessions, and other on the job training.
- Builds relationships and open communication channels with team members that foster trust and respect.
- Monitor random medication passes and oversight to ensure quality of care to clients.
- Provide insight regarding potential admissions, transfers, discharges, hospitalizations, and/or long-term care.
- Establish policies in partnership with the Clinical and Medical Service Coordinator related to client care and medication training and support.
- Performs other duties as required and/or assigned.

REQUIREMENTS (Competencies, skills, abilities, experience, education necessary.) |
- Graduate of an accredited School of Nursing
- Must have current licensure as a Registered Nurse or advanced practice nurse in the State of Illinois.
- Possess two years of clinical registered professional nursing experience within the last five years, at least one of which shall preferably have developmental and/or physical disabilities.
- Must have DHS Nurse Trainer certification or be eligible to become a Nurse Trainer within the first 3 months of hire.
- Must have a valid driver's license.
