Duties & Responsibilities:
- Assist physicians and health care team members in understanding the significant social and emotional factors related to the health problems
- Participate in the development of the plan of treatment, prepares clinical and progress notes, works with the family, utilizes appropriate community resources.
- Participate in discharge planning and in-service programs and acts as a consultant to other agency personnel.
Qualifications:
- Bachelor's Degree in Social Work or related healthcare field required.
- Knowledgeable regarding community resources required.
- Excellent communication skills and ability to work in a collaborative team approach are required.
- SC License Bachelor of Social Work (LBSW) required.
- Licensed by the S.C. State Board of Social Work Examiners as a Licensed Baccalaureate Social Worker, Licensed Master Social Worker, or Licensed Independent Social Worker required.
Preferred Qualifications:
- One (1) year of hospital/health care experience preferred.
- A Master's Degree from a school of social work accredited by the Council on Social Work Education is preferred.
