What are the responsibilities and job description for the RN Home Health Case Manager position at Anointed Healthcare Group?
Anointed Healthcare is growing!! We have FULL TIME and PRN opportunities in several territories in DFW Metroplex. That does not mean you cover all of DFW! We work to add team members for each territory to allow you to spend more time with the patients and families we have the honor to serve.
Our greatest needs are currently in our South Dallas, Lancaster, Collin County territories, and Dallas territory for Full Time. We also have PRN needs in all of our DFW territories.

Responsibilities:
- Ability to provide skilled nursing assessment to patients.
- Provide emotional/therapeutic support to families during difficult times.
- Implement RNCM Plan of Care
- Provide education to patient, family, and caregivers on plan of care
- Timely documentation, compliance with all regulations and conditions of participation, attendance and contribution to Case Conference
Requirements:
- Valid Texas Nursing License (RN)
- Reliable vehicle with personal auto insurance.
- Home Health RNCM experience
- Wound Care
Nice to have:
- Experience documenting in electronic medical records systems - Kinnser/Wellsky
Job Type: Full-time
Pay: $88,000.00 - $90,000.00 per year
