As a Home Health RN you will:
- Provide direct care to the patients, including treatments and medications
- Develop, follow, and coordinate each patient's comprehensive assessment and plan of care
- Notify physician, when necessary, of changes in patient's condition
- Complete all necessary documentation according to regulations and company policies and procedures
Qualifications and Skills:
- Current PA RN license in good standing
- Minimum of one year home health experience
- Self-directed with excellent observational ability and effective communication skills
- Ability to complete OASIS Assessments both timely and accurately
- Valid driver's license with current auto insurance
